Notice of Inability to Achieve Pay Equity

The Application for Review Services is used to make a complaint if there has been a contravention of the Pay Equity Act or a contravention of an Order issued by the Pay Equity Office or the Pay Equity Tribunal.

This form is for use when filing an Application for Review Services only where an Applicant or Applicants wishes to remain anonymous and wishes to designate an agent to act on their behalf.

As required by the Pay Equity Act, as amended, the employer described below hereby notifies the Pay Equity Office that it has at least one female job class for which a pay equity comparison cannot be made within an establishment by either the job-to-job or proportional value methods.
